Change of Address for Schools and Training Facilities Licensed Under Chapter 493, Florida Statutes

A school or training facility license is valid only for the training site named on the license. You are required to notify FDACS in writing within 10 days after changing the location of your school or training facility. You can submit your notification to the address at the bottom of the page. Please provide the date that instruction will begin and the street address and city of the primary building in which classes will be conducted.

FDACS will examine the submission to determine if it is in continued compliance with the requirements of the law and will conduct inspections to assist the school in meeting compliance as necessary. Within 30 days of receipt of the submission, FDACS will provide an updated license recognizing the new location.
